## What happened

An activity timeline entry attached to an operation could not be persisted. The underlying operation is unaffected; only the audit row is missing.

## Likely causes

- The activity timeline entry for an operation could not be saved because of a database error.
- The entry contained data that could not be stored (for example an unsupported value in its metadata).

## How to resolve

1. Confirm that the underlying operation completed; only its timeline entry is missing.
2. If the entry repeats for the same type of operation, contact support@bizkithub.com with the log entry so the cause can be fixed.
